Percutaneous Nephrolithotomy continues to challenge and fascinate urologists hence it has evolved with the advancement of technology into smaller tracts with equally efficient stone management but less renal injury. The first super mini-PCNL in the country was performed in Jose R. Reyes Memorial Medical Center last February 4, 2016. The team was composed of Drs. Albert Aquino, Juan Godofredo Bardelosa, Paul Villegas, Arvin Alvarez, Josef Ibarra and Paul Nimrod Firaza. A French 12 tract was created to access and eradicate the kidney stones as shown in the pre-operative CT Stonogram. The entire procedure was completed in 75 minutes. Complete stone clearance was confirmed in the post operative KUB xray. It was totally tubeless (no nephrostomy tube nor ureteral stent placed) with skin closure using steri-strips. The patient was discharged without complications on the 3rd post-op day.
